How to Apply

The application deadline for the 2012-2013 school year has passed. Please contact the Admissions Office for information about our waiting list before submitting an application.

An application is required for each individual applicant. Incomplete applications will not be considered. A complete application consists of the following items:

  • Completed Admissions Application form
  • $175.00 non-refundable application fee
  • Parish Verification Form (to be completed by your pastor for Catholic applicants only)
  • Confidential Teacher Questionnaire (to be completed by the current teacher)
  • Copies of Standardized Testing for the last two years (if applicable)
  • Copies of Report Cards for the last two years (if applicable)
  • Copies of birth certificate and baptismal certificate
  • Student Questionnaire (Grades 6-8 only)
  • Final transcript to be submitted upon completion of current school year

General Admissions Information from the Archdiocese of Atlanta

Queen of Angels Catholic School admits students of any race, sex, color, national, or ethnic origin to all rights, privileges, programs, and activities generally accorded or made available to all students at the school. We do not discriminate on the basis of race, sex, color, national and ethnic origin in administration of educational policies, admissions policies, loan programs, and other school administered programs.

Georgia State Law is followed for beginning age requirements for school entrance. For kindergarten, a child must be 5 years old on or before September 1st. Local transfer of students from any public, private, or Archdiocesan school into Grade 8 is not considered. Students whose families have relocated to the Atlanta area may apply for admission into Grade 8.

Students must also meet state standards for immunization. There is no provisional admission. For medical exemptions, a physician may indicate this on the form. The Catholic Church does not recognize an exemption based upon matter of conscience/religious exemption.
